Legal Notes by State
Rabies certificates: All states require a current rabies vaccination certificate. Keep the original and include a copy in this document. Many boarding facilities and groomers require it on file.
Heartworm prevention: Year-round prevention is recommended in all U.S. states by the American Heartworm Society. Document monthly preventative dosing here.
Interstate travel: If you cross state lines with your pet, a Certificate of Veterinary Inspection (CVI or health certificate) is often required โ separate from this document, but your medical history makes obtaining it much faster.
